1. Get quest that requires mob drops.
2. Aggro a ton of them.
3. Loot with AOE loot on
4. Possibly low quest item drop.
This is much more of a “feeling” so I could be wrong. Bad luck does happen. But I’ve seen it more than a few times now to think it should at least be checked.
